If you are a family affected by spina bifida in need of help with an additional financial burden or financial help to enjoy some much needed quality time, you can apply for a scholarship experience using the form on this page. Having a loved one with spina bifida impacts the entire family unit. We are here to provide scholarships to eligible and approved families to help build lasting memories, increase quality, and maybe even add a little extra fun into your lives! Please note, by submitting an application, you acknowledge that your application is subject to these terms and conditions and that, by applying, you agree to and will be bound by all such terms and conditions.

In order to receive the scholarship experience award you are applying for, you need to meet all of the following eligibility requirements:

  1. Have spina bifida or be an immediate family member or caregiver for someone with spina bifida.

  2. Live in the United States.

  3. Be willing to provide the information and documentation, upon request of Ellavation, to verify eligibility.

  4. Be willing to share images of your child and your family, and information about your scholarship experience, to be used in our marketing and in other organizational materials.

  5. Have not previously received a scholarship experience from Ellavation in the past 18 months.

In addition, applicants will be required to submit valid proof of US residency (such as a valid driver’s license or state ID), proof of spina bifida diagnosis, and an image of your child or family.  Needs for documentation and information may be subject to change, and Ellavation may, at any time, request additional information or documentation as it deems necessary to verify eligibility, assess need, prevent fraud, or comply with legal or audit requirements. Failure to timely provide requested information may result in denial or withdrawal of a scholarship award.

Important benefits notice. An applicant’s approval and receipt of funds from Ellavation could impact government assistance or other benefits. Ellavation makes no representations or warranties as to whether such assistance or benefits may be affected. It is the applicant’s responsibility to investigate any such impacts. By submitting an application, the applicant acknowledges and agrees that Ellavation is not responsible for any effect on the applicant’s assistance or benefits, and, to the fullest extent permitted by applicable law, the applicant hereby waives, releases, and forever discharges Ellavation and its officers, directors, employees, and agents, from any and all claims, demands, liabilities, costs, and expenses, whether known or unknown, arising out of or relating to any alleged interference with, reduction of, or ineligibility for any assistance or benefits resulting from the applicant’s approval for, or receipt or use of, funds from Ellavation. Nothing in this notice is intended to, or shall, waive any rights that cannot be waived under applicable law.

Participants will be selected at the discretion of Ellavation. Ellavation takes into account a number of factors, such as eligibility criteria and available funds, in making its decisions. In the event that the number of applicants exceeds the funds available, we may also ask income based questions to be used to determine awards of scholarship experiences.

The foregoing notwithstanding, all application reviews and award decisions are made by Ellavation in its sole and absolute discretion, and Ellavation reserves the right to decline any application for any reason permitted by law and makes no commitment to award any particular number or amount of scholarship experiences.

No Guarantee. Application does not guarantee an award of a scholarship experience. Awards, if granted, are personal, nontransferable, and may be subject to conditions, limitations, or deadlines set by Ellavation.

Acknowledgment. By submitting an application, you represent that all information provided is true and complete; you authorize Ellavation to verify information provided; and you agree to these guidelines and any additional terms communicated by Ellavation.

Scholarship experiences are limited to immediate family members in the same household and one additional caregiver.[1] 

Ellavation treats application information as confidential and will not disclose it except to its personnel, volunteers, service providers, advisors, insurers, or auditors who need to know such information and are subject to confidentiality obligations; to comply with law, regulation, subpoenas, or other legal processes; in connection with audits, disputes, or investigations; or as otherwise required for legal compliance.

Any applicant who submits an application grants Ellavation a worldwide, perpetual, royalty‑free license and permission to use the applicant’s and, as applicable, the child’s and immediate family’s name, story, likeness, image, voice, photographs, videos, and biographical information in Ellavation’s marketing, promotional, and fundraising materials, and other organizational materials, in any form, without further notice, approval, or additional compensation.

Program Changes and Termination. Ellavation may modify, suspend, or terminate the program, these guidelines, or any aspect of an award at any time, in its sole discretion, with or without notice, to the extent permitted by law.

Limitation of liability. To the maximum extent permitted by law, Ellavation and its affiliates, directors, officers, employees, volunteers, and agents will not be liable for any indirect, incidental, special, consequential, exemplary, or punitive damages, or for any loss of profits, goodwill, data, or other intangible losses, arising out of or relating to the program, any application, or any award of a scholarship experience, even if advised of the possibility of such damages. In no event will Ellavation’s total liability to any applicant exceed the greater of one hundred dollars (US $100) or the actual value of funds or benefits provided to that applicant under the program.

Dispute resolution; arbitration; governing law. Any dispute, claim, or controversy arising out of or relating to these guidelines, the program, your application, or any award will be resolved exclusively by binding, individual arbitration administered by the American Arbitration Association under its Consumer Arbitration Rules then in effect. The seat and place of arbitration will be Cook County, Illinois. The arbitrator will have the authority to award all remedies available in court, subject to the limitations set forth in these guidelines. You and Ellavation agree to waive any right to a jury trial and to participate in a class action, class arbitration, or representative proceeding. Notwithstanding the foregoing, either party may seek (a) individualized relief in a small‑claims court of competent jurisdiction and/or (b) temporary, preliminary, or interim injunctive relief in any court of competent jurisdiction to protect confidential information or intellectual property rights. These guidelines and any dispute between you and Ellavation are governed by the laws of the State of Illinois, without regard to its conflicts‑of‑law principles. Judgment on the arbitration award may be entered in any court having jurisdiction. You consent to the exclusive jurisdiction of the state and federal courts located in Illinois for purposes of enforcing this arbitration agreement or any arbitral award.
